Summary
In order to submit your Subject Access Request
- Complete the Application Form which can be found at the bottom of this page
- Provide photocopies of your identification – proving your name, current address and date of birth
- Return the application to the Police and Crime Commissioners office at the below address
Once a valid Subject Access Request has been received, the Police and Crime Commissioners office are under a legal obligation to respond to the applicant within 1 month.
